Welcome to the collaboration between the Louisiana Office of Public Health’s Community HealthWays program and the Louisiana Rural Health Association! Together, we're increasing the use of community health workers (CHWs) in EMG-served parishes. EMG is committed to providing CHW services to all 22 parishes and schools, bridging critical health gaps. The CHW model reduces rural-urban health disparities, improves individuals' quality of life, and boosts community engagement in preventative health activities.
A CHWs is a trusted community member with an unusually close understanding of the community served and available resources. The Community Health Worker assists individuals in addressing social problems that lead to poor health with increased attention on food insecurity, housing, transportation, utility assistance, furthering education, mental health connections, and education related to chronic conditions. We do not provide any monetary assistance. We merely connect you with resources to try and help alleviate the issues you're having.
